- Heart attack
- Invasive cancer (excludes most skin cancer)
- Renal (kidney) failure
- Occupational HIV
- ALS (Lou Gehrig’s disease)
- Transplant of a major organ
- Paralysis of at least two limbs
- Carcinoma in situ (25% benefit)3
- Coronary artery by-pass surgery (25% benefit)3
